All Dawgs will evaluate your dog for food guarding issues. I understand that since my dog is on a restricted diet, I am required to bring 3-5 ounces of WET dog food that your dog may eat. I understand that if I do not bring in this food, All Dawgs will use the food typically used for these evaluations.

OFA Xrays*
By checking this box, you acknowledge you will have the dog have OFA x-rays after his/her second birthday at your own expense to confirm the dog is physically able to perform as a service dog. All Dawgs is to receive the results the OFA certificates before this task training will commence. This task will not be taught if this dog does not receive OFA certification of "Good" or "Excellent."*

Evaluation Recording*

By checking this box, you acknowledge that you understand there is a no cancellation policy for any aspect of Service Dawg training (including but not limited to the evaluation and private lessons.) This includes reschedules.*
